This form is designed to be easily understood and followed although should a member have concerns about filling the form in then they should contact their local delegate or Organiser for assistance.
The form assists the member and the Union in identifying the allegations and allows the Union to assist in resolving any allegation of Bullying and Harassment prior to the issue escalating.
